Information we collect and how we use it
We may collect the following types of information:
Log information – When you access The Treatment Center’s web pages via a browser, application or other client service, servers automatically record certain information. These server logs may include information such as your web request, your interaction with a service, Internet Protocol address, browser type, browser language, the date and time of your request and one or more cookies that may uniquely identify your browser or your account.
User communications – When you send email or other communications to The Treatment Center, we may retain those communications in order to process your inquiries, respond to your requests and improve our services. When you send and receive SMS messages to or from one of our services that provides SMS functionality, we may collect and maintain information associated with those messages, such as the phone number, the wireless carrier associated with the phone number, the content of the message, and the date and time of the transaction. We may use your email address to communicate with you about our services.
Location data – The Treatment Center offers location-enabled services (such as GPS signals sent by a mobile device) or information that can be used to approximate a location (such as a cell ID or IP address).
In addition to the above, we may use the information we collect to:
Provide, maintain, protect, and improve our services (including advertising services) and develop new services; and
Protect the rights or property of The Treatment Center or our patients.
If we use this information in a manner different than the purpose for which it was collected, then we will ask for your consent prior to such use.
The Treatment Center processes personal information on our servers in the United States of America and in other countries. In some cases, we process personal information outside your own country.
Most browsers are initially set up to accept cookies, but you can reset your browser to refuse all cookies or to indicate when a cookie is being sent. However, some features and services may not function properly if your cookies are disabled.
The Treatment Center uses the Google and Yahoo retargeting advertising cookies on AdSense partner sites and certain other services to help advertisers and publishers serve and manage ads across the web. You can view and manage your ads preferences associated with this cookie.
The Treatment Center only shares personal information with other companies or individuals outside of The Treatment Center in the following limited circumstances:
We have your consent. We require opt-in consent for the sharing of any sensitive personal information.
We have a good faith belief that access, use, preservation or disclosure of such information is reasonably necessary to (a) satisfy any applicable law, regulation, legal process or enforceable governmental request, (b) enforce applicable Terms of Service, including investigation of potential violations thereof, (c) detect, prevent, or otherwise address fraud, security or technical issues, or (d) protect against harm to the rights, property or safety of The Treatment Center, its users or the public as required or permitted by law.
We take appropriate security measures to protect against unauthorized access to or unauthorized alteration, disclosure or destruction of data. These include internal reviews of our data collection, storage and processing practices and security measures, including appropriate encryption and physical security measures to guard against unauthorized access to systems where we store personal data.
We restrict access to personal information to The Treatment Center employees, contractors and agents who need to know that information in order to process it on our behalf. These individuals are bound by confidentiality obligations and may be subject to discipline, including termination and criminal prosecution, if they fail to meet these obligations.
Accessing and updating personal information
When you use The Treatment Center services, we make good faith efforts to provide you with access to your personal information and either to correct this data if it is inaccurate or to delete such data at your request if it is not otherwise required to be retained by law or for legitimate business purposes. We ask individual users to identify themselves and the information requested to be accessed, corrected or removed before processing such requests, and we may decline to process requests that are unreasonably repetitive or systematic, require disproportionate technical effort, jeopardize the privacy of others, or would be extremely impractical (for instance, requests concerning information residing on backup tapes), or for which access is not otherwise required. In any case where we provide information access and correction, we perform this service free of charge, except if doing so would require a disproportionate effort. Because of the way we maintain certain services, after you delete your information, residual copies may take a period of time before they are deleted from our active servers and may remain in our backup systems. The Treatment Center complies with the U.S.-EU Safe Harbor Framework and the U.S.-Swiss Safe Harbor Framework as set forth by the U.S. Department of Commerce regarding the collection, use, and retention of personal information from European Union member countries and Switzerland. Google has certified that it adheres to the Safe Harbor Privacy Principles of notice, choice, onward transfer, security, data integrity, access, and enforcement. NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW THIS CAREFULLY.
Uses and Disclosures:
Treatment. Your health information may be used by staff members or disclosed to other health care professionals for the purpose of evaluating your health, diagnosing medical conditions, and providing treatment. For example, results of laboratory tests and procedures will be available in your medical record to all health professionals who may provide treatment or who may be consulted by staff members.
Payment. Your health information may be used to seek payment from your insurance company(s) (health, dental, auto or other) and from any other source that you may use to pay for services such as credit card, cashier check, personal check and or banking company(s). For example, your health plan may request and receive information on dates of service, the services provided, and the medical condition being treated.
Health care operations. Your health information may be used as necessary to support the day-to-day activities and management of The Treatment Center. For example, information on the services you received may be used to support budgeting and financial reporting, and activities to evaluate and promote quality.
Law enforcement. Your health information may be disclosed to law enforcement agencies, without your permission, to support government audits and inspections, to facilitate law-enforcement investigations, and to comply with government mandated reporting.
Public health reporting. Your health information may be disclosed to public health agencies as required by law. For example, we are required to report certain communicable diseases to the state's public health department.
Other uses and disclosures require your authorization. Disclosure of your health information or its use for any purpose other than those listed above requires your specific written authorization. If you change your mind after authorizing a use or disclosure of your information you may submit a written revocation of the authorization. However, your decision to revoke the authorization will not affect or undo any use or disclosure of information that occurred before you notified us of your decision.
Additional Uses of Information
Payment reminders: Our staff will use your health information to send you payment reminders.
Information about treatments: Your health information may be used to send you information on the treatment and management of your medical condition that you may find to be of interest. We may also send you information describing other health-related goods and service that we believe may interest you.
You have certain rights under the federal privacy standards. These include:
• The right to request restrictions on the use and disclosure of your protected health
• The right to receive confidential communications concerning your medical condition and
• The right to inspect and copy your protected health information
• The right to amend or submit corrections to your protected health information
• The right to receive an accounting of how and to whom your protected health information has been disclosed
• The right to receive a printed copy of this notice
The Treatment Center Duties
We are required by law to maintain the privacy of your protected health information and to provide you with this notice of privacy practices. We also are required to abide by the privacy policies and practices that are outlined in this notice.
Right to Revise Privacy Practices
As permitted by law, we reserve the right to amend or modify our privacy policies and practices. These changes in our policies and practices may be required by changes in federal and state laws and regulations. Whatever the reason for these revisions, we will provide you with a revised notice on your next office visit. The revised policies and practices will be applied to all protected health information that we maintain.
Requests to Inspect
Protected Health Information
As permitted by federal regulation, we require that requests to inspect or copy protected health information be submitted in writing. You may obtain a form to request access to your records by contacting Medical Records or The Privacy Officer. If you believe that your privacy rights have been violated, you should call the matter to our attention by sending a letter describing the cause of your concern to the same address. You will not be penalized or otherwise retaliated against for filing a complaint.

SOCIAL MEDIA ENGAGEMENT DISCLAIMER
You expressly acknowledge that you assume all responsibility related to the security, privacy, and confidentiality risks inherent in sending any content over the internet. By its very nature, a website and the internet cannot be absolutely protected against intentional or malicious intrusion attempts. The Treatment Center does not control the third party sites and the internet over which you may choose to send confidential personal or health information or other content and, therefore, The Treatment Center does not warrant safeguard against any such interceptions or compromises to your information. When posting any content on an internet site, you should think carefully about your own privacy in disclosing detailed or private information about yourself and your family. Furthermore, The Treatment Center does not endorse any product, services, views or content displayed by others on The Treatment Center social media sites.
By posting any comments, posts or other material on The Treatment Center’s blogs or social media sites, you give The Treatment Center the irrevocable right to reproduce, distribute, publish, display, edit, modify, create derivative works from, and otherwise use your submission for any purpose in any form and on any media.
You agree that you will indemnify The Treatment Center against any damages, losses, liabilities, judgments, costs or expenses (including attorney’s fees and costs) arising out of a claim by a third party relating to any material you have posted.
When you post a comment to a social media site, including but not limited to: Twitter, Facebook, blogs, multimedia or other user-generated content, it is published for the world to see. For your own privacy and that of your family, you should consider how much detailed personal and medical information is linked to your name.
Information gathered by third party applications may be used for marketing purposes within The Treatment Center. Information will not be sold or shared.
This policy may be updated at any time without notice, and each time a user accesses a Treatment Center social networking site, the new policy will govern, usage, effective upon posting.
